Patch Party Minor Guest Waiver Form

Waiver form for your minor children that will be attending this party.

, being the parent or legal guardian of

and being fully informed of the scope of activities in which Minor will participate and to which Minor may or will be exposed, consent to Minor’s participation in party activities (“Activities”) with Shine Bright, Sis! LLC. In connection with the Activities, Minor may be exposed to craft glue and fabric markers, contact with other participants, residents of the location of the party, Shine Bright, Sis! personnel, and other hazards. Guardian and Minor have had the opportunity to ask questions of Shine Bright, Sis! regarding the Activities and wish to have Minor participate in the Activities. Guardian and Minor voluntarily assume the risks associated with the Activities, and any hazardous conditions related thereto, to the full extent allowed by Arizona law. In consideration of Shine Bright, Sis! permitting the Minor to engage in the Activities, Guardian and Minor hereby release and forever discharge Shine Bright, Sis! and it’s owners, employees, agents, and representatives (collectively, “Shine Bright, Sis! LLC”) from all claims, actions, and/or liabilities based on or arising from injury, loss, damage or death to Guardian or Minor, or injury, loss or damage to any property owned by Guardian or Minor, which is caused by or sustained in connection with the Minor’s participation in the Activities including negligence in training, maintenance, care, supervision, conditions to which the Minor is exposed and/or the supervision of the other participants. Guardian and Minor agree to abide by and follow any instructions given and/or rules established by Shine Bright, Sis! regarding the Activities. Guardian and Minor shall indemnify Shine Bright, Sis! for the defense of any such claims and/or actions (including reasonable attorneys’ fees and costs), and for any liabilities based on or arising from such claims and/or actions.

Please check whether you do or not consent to giving Shine Bright, Sis! permission to use photographs taken at this event that may have your child in the pictures. By giving permission, you are allowing Shine Bright, Sis! to use photographs taken at this event for any legal uses, including but not limited to publicity, social media, web content, and advertising. Furthermore, by giving permission you understand that no royalty, fee, or other compensation shall become payable to you for any reason of such use.
